Terms and Conditions for accessing to "real time Live Surgery via web cast"
By registering to be viewers of the "Web Cast of Live Surgery" of this Symposium, the users acknowledge to have read and agree with the terms and conditions stated below:
1. Real time web cast of live surgery will be available via Windows machines. Login details for viewing the live surgery will be sent to successful registered users via email one week prior to the event date.
2. The organizers cannot be made accountable for and do not bear any responsibility for any technical / access error which may arise out of the organizers’ control during broadcast. Under no circumstances should the organizers, directors or employees be liable to any claims for compensation for direct, indirect or consequential loss or damages arising out of or in connection with failure to access or interruption of the broadcast.
3. The “Web Cast of Live Surgery” is intended only for those who have registered and subscribed to the service. The transmission is not for use / distribution to any other third parties who are not subscribers of this service.
4. No part of the transmission is to be recorded / reproduced / modified / distributed. The broadcasted material is the copyright of the organizers.
 
Registration Entitlements (Attending Symposium)
Delegates registered to attending the Symposium are entitled to joining the lecture sessions and watching the live surgery. Refreshments will be served during breaks and lunch will provided on Day 1.
 
Payment
Payment can be made by credit card or by bank draft / cheque payable to "The Chinese University of Hong Kong". It should be sent together with the completed registration form to the Secretariat. (Personal cheques are acceptable for Hong Kong residents only.)
 
Cancellation & Refund Policy

Notification of cancellation must be made in writing to the Secretariat. For cancellation request received on or before 6th February, 2015, 50% of the registration fee will be refunded. No refund will be made for cancellation received after 6th February, 2015.

 
Private Policy Statement
The personal data provided by registrants / applicants will be used by the Department of Surgery, CUHK for the purposes of processing registration to the workshops / conferences and delivering information of current and future events. The data will not be transferred to other external parties except for the co-organizers of events for communication of programmes / events purpose. Under the provision of the Personal Data (Privacy) Ordinance, registrants / applicants have the rights to access to and request the correction of the personal data.
