Who Need Surgery
As the main purpose of bariatric surgery is to control and reduce the risk of suffering from obesity-related
disease and hence reduce the chance of premature death, the criteria for surgery is dependent on balance between
health risk from obesity and likelihood of benefit from surgery.
From the experience from western countries, most countries follow the guideline from the Consensus Statement from
National Institute of Heath of USA in 1991:

Morbid obesity

Failure of
previous medical weight reduction treatment

Age 18 – 55

| 4. |
No endocrine
cause of obesity (mainly exclude Hypothyroidism and Cushing Disease)

No major
psychiatric diseases (mainly exclude schizophrenia and uncontrolled major depression)
|
| 6. |
No substance
abuse & alcoholic
|
| 7. |
No major
organ failure which leads to contraindication for general anesthesia
|
DEFINITION OF MORBID OBESITY FOR SURGERY
In Western countries, the Morbid Obesity is defines as:
1. |
BMI ≥ 40 kg/m2, or
|
2. |
BMI ≥ 35 kg/m2 with severe obesity-related morbidities
|
However, Asians are more prone to develop obesity-related disease than Caucasian and the WHO had defined a lower
BMI of 25 kg/m2 as obesity in Asian (as compared of 30kg/m2 in Caucasian).
In Asia-Pacific Region, we define Morbid Obesity according to the guideline from the Consensus Statement
from Asia-Pacific Bariatric Surgeons Group in 2005:

BMI ≥ 37 kg/m2, or

BMI ≥ 32 kg/m2 plus
Type 2 Diabetes or two obesity-related co-morbidities
