INTRODUCTION

The Division of Upper Gastrointestinal and Metabolic Surgery (Team II) is world renowned for its research in the surgical treatment of a great variety of upper gastrointestinal tract diseases, endocrine disorders and metabolic diseases associated with obesity. In conjunction with the Endoscopy Centre, we are also a leading unit in advanced Endoscopic Retrograde Cholangiopancreatography (ERCP), Endoscopic Ultrasound (EUS), submucosal endoscopy and the management of upper gastrointestinal bleeding.

SERVICES

The Division of Upper Gastrointestinal and Metabolic Surgery (Team II) provides elective and emergency general surgery services for the New Territory East Cluster (NTEC) population in Hong Kong. We also provide subspecialty support for Hong Kong as a Territory referral centre of Upper Gastrointestinal (GI) surgery and advanced therapeutic endoscopy. We are one of the recognized sub-speciality training centres on Upper GI Surgery by the Hong Kong College of Surgeons. Moreover, our Metabolic & Bariatric Surgery centre provides multi-disciplinary team services for patients with morbid obesity and uncontrolled diabetes mellitus. In addition, our Endocrine unit also provides surgical management of various endocrine disorders including neuroendocrine tumour, thyroid, parathyroid and adrenal diseases. In collaboration with our Gastroenterologists, our Endoscopy Centre provides 24 hours services in endoscopic management of GI diseases which include advanced therapeutic upper endoscopy (OGD), Colonoscopy, ERCP and EUS.

  • Epigenetic changes in squamous cell carcinoma of the oesophagus.
  • Electro-acupuncture for gastro-oesophageal reflux diseases.
  • Efficacy and safety of laparoscopic anti-reflux procedures for GERD in Chinese.
  • Trans-arterial embolisation for high risk bleeding peptic ulcer.

Advanced Therapeutic Endoscopy:

  • Over-the-Scope Clip for upper gastrointestinal bleeding.
  • Anti-reflux Mucosectomy (ARMS) for gastro-oesophageal reflux disease.
  • Submucosal endoscopy.
  • Therapeutic interventional EUS.
  • Endoscopic sleeve gastroplasty (ESG) for morbid obesity.

Metabolic & Bariatric Surgery:

  • Sleeve gastrectomy with loop duodenojejunostomy versus Roux-en Y gastric bypass in obese uncontrolled type 2 diabetes.
  • Prophylactic Anterior Crural repair in prevention of De Novo gastroesophageal reflux after sleeve gastrectomy.
  • Clinical course and biomarkers associated with nonalcoholic fatty liver disease (NAFLD) in Chinese patients with obesity.
  • Integrated genetic, epigenetic and clinical trait-based model for predicting metabolic surgery responses.

ACHIEVEMENTS

The Team has established itself as a world leading centre in research & our publications are regularly featured in top grade journals. Not only are our publications widely cited, their findings have made major impact in clinical practices:

  1. Endoscopic therapy for upper GI bleeding
    • Injection therapy of bleeding peptic ulcers.
    • Refinement of endoscopic techniques in treatment of bleeding oesophageal varices.
    • Clinical trials in the use of antibiotics alone against Helicobacter pylori associated peptic ulcers without acid suppression.
  2. Advance endoscopic therapy for GI diseases in Hong Kong
    • Endoscopic submucosal dissection (ESD) for GI early cancers.
    • Peroral Endoscopic Myotomy (POEM) for achalasia.
    • Advanced ERCP techniques.
    • EUS-guided interventions including drainage of pancreatic fluid collections, biliary and gallbladder drainage, gastroenterostomy and other interventions.
  3. Surgical & endoscopic therapy for obesity & diabetes in Hong Kong
    • Introduction of gastric band, sleeve gastrectomy and gastric bypass surgery.
    • Introduction of endoscopic bariatric therapy - intragastric balloon & endoscopic sleeve gastroplasty.
  4. Advancement of Minimally Invasive treatment for Endocrine Diseases
    • CT-guided radiofrequency ablation in treatment of benign adrenal neoplasms.
    • Endoscopic / Robotic / Transoral thyroidectomy for thyroid neoplasms.

Apart from clinical research, our Team provides Continued Medical Education through organization of local, regional & international workshops.

  • Since 1985, our International Therapeutic Endoscopy Workshop has attracted hundreds of participants from around the world each year to join.
  • We have organised hundreds of workshops in minimally invasive and robotic surgery leading to propagation of the technique in the Asian-Pacific region.
